ما هي البواسير؟

البواسير هي أوردة متورمة ومتضخمة في منطقة الشرج والمستقيم (نهاية الأمعاء الغليظة). يمكن أن تكون داخلية (داخل المستقيم) أو خارجية (تحت الجلد حول فتحة الشرج). البواسير شائعة جدًا ويمكن أن تسبب عدم الراحة والألم والحكة والنزيف.

أسباب ظهور البواسير:

  • الضغط الزائد في منطقة الشرج: مثل الإجهاد أثناء التبرز، والإمساك المزمن، والإسهال المزمن، والحمل، والسمنة، ورفع الأشياء الثقيلة.
  • التقدم في السن: مع التقدم في السن، تصبح الأنسجة المحيطة بالشرج أضعف.
  • الوراثة: يكون الأشخاص الذين لديهم تاريخ عائلي للإصابة بالبواسير أكثر عرضة للإصابة بها.
  • الجلوس لفترات طويلة: الجلوس لفترات طويلة، خاصة على الأسطح الصلبة.
  • اتباع نظام غذائي قليل الألياف: يمكن أن يؤدي اتباع نظام غذائي قليل الألياف إلى الإمساك والإجهاد أثناء التبرز.

أعراض البواسير:

  • النزيف: نزيف غير مؤلم أثناء التبرز، وقد يظهر على شكل قطرات دم على البراز أو في المرحاض.
  • الألم والانزعاج: خاصة في البواسير الخارجية أو المتخثرة (البواسير التي تكونت فيها جلطة دموية).
  • الحكة: في منطقة الشرج.
  • الشعور بوجود كتلة أو انتفاخ: في منطقة الشرج، خاصة في البواسير الخارجية.
  • الشعور بعدم الإفراغ الكامل: بعد التبرز.

علاج البواسير:

يعتمد علاج البواسير على شدة الأعراض ونوعها. تشمل طرق العلاج ما يلي:

  1. العلاجات المنزلية وتغيير نمط الحياة:
    • تناول كمية كافية من الألياف: لتليين البراز ومنع الإمساك.
    • شرب كمية كافية من الماء: للحفاظ على رطوبة الجسم وتليين البراز.
    • تجنب الإجهاد أثناء التبرز.
    • استخدام الجلوس في الماء الدافئ (حمام المقعدة): لتخفيف الألم والالتهاب.
    • استخدام الكمادات الباردة: لتقليل التورم.
    • تجنب الجلوس لفترات طويلة.
    • تجنب تناول الأطعمة الحارة والمثيرة.
  2. العلاجات الدوائية:
    • الكريمات والمراهم الموضعية: لتقليل الألم والحكة والالتهاب.
    • التحاميل المضادة للالتهابات: لتخفيف أعراض البواسير الداخلية.
    • مسكنات الألم: لتخفيف الألم.
    • الملينات: لمنع الإمساك.
  3. العلاجات غير الجراحية:
    • ربط الشريط المطاطي: للبواسير الداخلية، حيث يتم ربط قاعدة الباسور بشريط مطاطي مما يؤدي إلى جفافه وضموره.
    • المعالجة بالتصليب: حقن محلول كيميائي داخل الباسور لتقليصه.
    • التخثير بالليزر أو الأشعة تحت الحمراء: لتقليص البواسير.
  4. العلاجات الجراحية:
    • استئصال البواسير: جراحة لإزالة البواسير، وتستخدم للبواسير الكبيرة أو المؤلمة أو التي لا تستجيب للعلاجات الأخرى.
    • تثبيت البواسير بالتدبيس: طريقة لجراحة البواسير الداخلية حيث يتم سحب نسيج الباسور إلى داخل الشرج باستخدام جهاز التدبيس.

الرعاية بعد العلاج:

  • تناول كمية كافية من الألياف والماء.
  • تجنب الإجهاد أثناء التبرز.
  • استخدام مسكنات الألم التي يصفها الطبيب.
  • الحفاظ على نظافة منطقة الشرج.

What are Jaw Tumors?

Jaw tumors are abnormal growths of cells in the jawbone (upper or lower) or surrounding tissues. These tumors can be benign (non-cancerous) or malignant (cancerous). Benign tumors usually grow slowly and do not invade surrounding tissues, while malignant tumors can grow rapidly and spread to other parts of the body.

Types of Jaw Tumors:

  1. Benign (Non-Cancerous) Tumors:
    • Ameloblastoma: The most common benign tumor of the jawbone, usually grows slowly and can cause swelling and deformation of the jaw.
    • Odontoma: Benign dental tumors that arise from tooth-forming cells.
    • Myxoma: A benign tumor with mucous tissue, more commonly seen in the lower jaw.
    • Osteoma: A benign bone tumor that usually grows on the surface of the bone.
    • Odontogenic Cysts: Cysts that originate from tooth-forming tissues.
  2. Malignant (Cancerous) Tumors:
    • Squamous Cell Carcinoma: The most common jaw cancer, usually starts from cells on the surface of the mouth or gums and can spread to the jaw.
    • Osteosarcoma: Cancer of the jawbone, usually seen in younger people.
    • Chondrosarcoma: Cancer of the cartilage that can develop in the jaw.
    • Metastatic Tumors: Cancers that have spread from other parts of the body (such as lung, breast, prostate) to the jaw.

Symptoms of Jaw Tumors:

  • Swelling or a mass in the jaw
  • Pain or discomfort in the jaw
  • Numbness or tingling in the lip or chin
  • Loose teeth
  • Difficulty opening and closing the mouth
  • Change in facial shape
  • Sore or bleeding gums that do not heal

Diagnosis of Jaw Tumors:

  • Clinical Examination: Examination of the jaw and mouth by a doctor or dentist.
  • Imaging: Including X-rays of the teeth and jaw, CT scans, MRI, and PET scans.
  • Biopsy: Taking a tissue sample for microscopic examination and to determine the type of tumor.

Treatment of Jaw Tumors:

The type of treatment depends on the type of tumor, its size, location, and the patient’s overall condition.

  1. Surgery:
    • Tumor Removal: Involves removing the tumor and surrounding tissue (with a safe margin).
    • Jaw Reconstruction: Using bone grafts or tissues to reconstruct the jaw after tumor removal.
    • Minimally Invasive Surgery: Surgical procedures with smaller incisions.
    • Robotic Surgery: Using robots for greater precision in surgery.
  2. Radiation Therapy (Radiotherapy): Using radiation to destroy cancer cells, alone or in combination with surgery.
  3. Chemotherapy: Using chemical drugs to destroy cancer cells, usually for malignant or metastatic tumors.
  4. Targeted Therapy: Using drugs that specifically target cancer cells.
  5. Immunotherapy: Using the body’s immune system to fight cancer.

Post-Treatment Care:

  • Regular Follow-Up: Regular check-ups to check for tumor recurrence.
  • Physical Therapy: Exercises to restore jaw function.
  • Proper Nutrition: Consuming nutritious foods for recovery.
  • Dental Care: Examination and care for dental health.
  • Psychological Support: Counseling and support to cope with stress and anxiety caused by the disease.

 

What is Colon Cancer?

Colon cancer is a type of cancer that forms in the colon (the large intestine), the lower part of the digestive system. It typically starts as benign polyps (small growths) on the lining of the colon. If these polyps are not detected and treated early, they can become cancerous.

Treatment Methods for Colon Cancer:

Treatment for colon cancer depends on the stage of the cancer, the location of the tumor, the patient’s overall health, and other factors. The main treatment options include:

  1. Surgery:
    • Tumor Removal: Surgery is the primary treatment for colon cancer, and the goal is to remove the cancerous tumor and surrounding tissues.
    • Colectomy: In this surgery, the portion of the colon affected by cancer is removed, and if necessary, the surrounding lymph nodes are also removed.
    • Hemicolectomy: If the cancer is located in one section of the colon, only that section (e.g., right or left hemicolectomy) may be removed.
    • Anterior or Posterior Resection: Depending on the location of the tumor, surgery may be performed in different parts of the colon and rectum (the end of the large intestine).
    • Ostomy: In some cases, if the removed part of the intestine cannot be reconnected, a temporary or permanent ostomy (creating an opening on the abdominal surface for stool output) may be performed.
  2. Chemotherapy:
    • Chemotherapy uses medications that destroy cancer cells. This treatment may be used before surgery (neoadjuvant) to shrink the tumor, after surgery (adjuvant) to eliminate remaining cancer cells, or as the primary treatment for advanced cancers.
    • Chemotherapy is usually given intravenously.
  3. Radiation Therapy:
    • Radiation therapy uses high-energy beams to destroy cancer cells. It is more commonly used for rectal cancer (the end of the large intestine) and less often for colon cancer.
    • Radiation therapy may be given before or after surgery or along with chemotherapy.
  4. Targeted Therapy:
    • These treatments focus on specific proteins and genes of cancer cells. The goal of these drugs is to stop the growth and spread of cancer cells.
    • Targeted therapies are usually used for advanced cancers or cancers that have not responded to other treatments.
  5. Immunotherapy:
    • Immunotherapy is a type of treatment that stimulates the body’s immune system to fight cancer.
    • This treatment may be effective in some patients with advanced colon cancer.

Post-Treatment Care:

  • Regular Follow-up: After colon cancer treatment, regular follow-up with the doctor is necessary to check for potential recurrence of the disease and perform the necessary tests.
  • Proper Nutrition: A healthy and balanced diet after surgery and other treatments is important for recovery and maintaining health.
  • Physical Activity: Regular physical activity helps improve the patient’s physical and mental well-being.
  • Psychological Support: Receiving psychological support and counseling to cope with the stress and anxiety caused by cancer diagnosis and treatment is very important.

 

What is Laryngeal Cancer?

Laryngeal cancer is a type of cancer that develops in the larynx (voice box). The larynx is an organ in the neck responsible for producing sound, breathing, and swallowing. Laryngeal cancer usually begins in the lining cells of the larynx. This cancer is more common in smokers and heavy alcohol users.

Symptoms of Laryngeal Cancer:

  • Hoarseness or voice change lasting more than two weeks.
  • A lump or bump in the neck.
  • Persistent sore throat.
  • Difficulty swallowing (dysphagia).
  • Chronic cough.
  • Ear pain.
  • Shortness of breath or wheezing.
  • Unexplained weight loss.

Diagnosis of Laryngeal Cancer:

  • Clinical Examination: Examination of the neck and larynx by a doctor.
  • Laryngoscopy: Examination of the larynx using a thin, flexible tube with a camera (laryngoscope).
  • Biopsy: Taking a sample of suspicious tissue to accurately diagnose the type of cancer.
  • Imaging:
    • CT Scan (Computed Tomography): To examine the extent of the cancer into surrounding areas.
    • MRI (Magnetic Resonance Imaging): To more thoroughly examine the soft tissues.
    • PET Scan (Positron Emission Tomography): To examine the spread of cancer to other parts of the body.

Treatment of Laryngeal Cancer:

The type of treatment depends on the stage of cancer, the size of the tumor, its location, and the patient’s overall condition.

  1. Surgery:
    • Tumor Removal: Surgical removal of the tumor.
    • Laryngectomy: Removal of part or all of the larynx.
      • Partial Laryngectomy: Removing a part of the larynx, which allows for the preservation of voice.
      • Total Laryngectomy: Removing the entire larynx, resulting in loss of voice and requiring the use of other communication methods such as talking with the aid of an electrolarynx.
    • Neck Dissection: Removal of lymph nodes in the neck if they are involved.
  2. Radiation Therapy (Radiotherapy):
    • Using radiation to destroy cancer cells, often before or after surgery.
  3. Chemotherapy:
    • Using chemical drugs to destroy cancer cells, usually combined with radiation therapy.
  4. Targeted Therapy:
    • Using drugs that specifically target cancer cells.
  5. Immunotherapy:
    • Using the body’s immune system to fight cancer cells.

Post-Treatment Care:

  • Regular Follow-Up: Regular check-ups to check for cancer recurrence.
  • Speech Therapy: To restore voice and improve speech after a laryngectomy.
  • Physical Therapy: To improve the movement of the neck and shoulder.
  • Proper Nutrition: To help in recovery and strengthen the immune system.
  • Psychological Support: Counseling and support to cope with stress and anxiety caused by the disease.
